Licensing in another province

If you are a dental assistant registered with the MDA who is applying for registration and licensure in another province or territory and require a Letter of Standing, complete and submit the Consent for Release of Information Form, along with the $100.00 (+GST) processing fee to the attention of: "Manitoba Dental Association."
